Credentialed Diabetes Educators are healthcare practitioners qualified to provide client-centred approach to diabetes education and care, empower clients and assist them in dealing with daily self-management, in accordance with the National Standards of practice for Diabetes Educator.

To provide our community with the access to education and information on Diabetes the Edenhope District Memorial Hospital has a Credentialed Diabetes Educator.


This role includes but is not limited to:

  • Information on the latest knowledge and techniques in diabetes health care and education.
  • Interviewing and counseling clients and/or their carers.
  • Assessment of patient/client needs.
  • Able to plan, and conduct quality diabetes education programs tailored to the needs of the client with diabetes.
  • Education of staff regarding best practice in diabetes management.
  • Diabetes management for both inpatients and outpatients.
  • A focus on health promotion and health prevention.
  • The role also includes assessing, planning and evaluating acute in-patients and aged care residents in the Kowree Nursing Home and Lakes Hostel.

The diabetes educator is available to see clients on an outpatient basis in the Edenhope Hospital Medical Clinic.
